In order for families to receive Commonwealth assistance for childcare, you need to provide information regarding the immunisation status of your child when enrolling for care.
- Immunisation History Statement obtained from Australian Childhood Immunisation Register (ACIR)
- An Immunisation History Form IMMU13 (that shows your child is on a catch up schedule)
- Documentation that shows your child has a medical reason not to be immunised
- NSW vaccination objection form
Your child can still attend Albury Family Day Care if they are not immunised or if they are on a 'catch up' plan. However if there is an outbreak of a vaccine preventable disease your child will need to be excluded from care until the required exclusion period has passed.
Parents who do not fully immunise their children up to 19 years of age will not be eligible for Commonwealth assistance for childcare unless their child has medical contradictions or natural immunity for certain diseases or is on a recognised catch up schedule or has completed and lodged a NSW vaccination objection form.
