Reset Your Life is a FREE lifestyle modification program targeting people aged between 40-49 years at high risk of developing type 2 diabetes. The group program helps participants make lifestyle changes that will ultimately prevent or delay the onset of type 2 diabetes.

"At high risk" of developing type 2 diabetes is determined following the person's completion of the Australian Type 2 Diabetes Download Risk_Assessment2.pdf Risk Assessment tool (653KB) (AUSDRISK) and a score of 12. This tool measures type 2 diabetes risk factors which include a family history of diabetes, high blood glucose levels, high blood pressure, smoking, unhealthy diet, less than 2.5 hours of physical activity per week and/or a waist measurement of >90cm for women and >100cm for men.

Reset Your Life involves 7 sessions of 1˝ - 2 hour duration which cover:

  •      Type 2 diabetes; including prevention, risk factors, how it is diagnosed and treated
  •       Healthy eating
  •       Physical activity
  •       How to make healthy lifestyle choices
  •       Setting goals to improve your health
  •       Stress management

Program Eligibility

To register for this program participants must be aged between 40 and 49 years, score 12 or more on the AUSDRISK tool and visit their GP for a blood glucose test and to receive a referral to participate in the program.

** those 50 years and over can participate in a similar program called Life! Taking action on Diabetes. This program is run by Health and Fitness Plus in Albury, T: 02 6045 8191.

A free Reset Your Life course will be run at Gateway Community Health commencing Wednesday 4 May 2011. Sessions will be held after hours (5:30pm start), with weekly sessions for the first 3 weeks, followed by 3 fortnightly sessions and a final followup session six months later.  More information can be obtained from The Health Promotion Team at Gateway Community Health.
